Responsibilities:

  • Undertake timely physiotherapy assessments on inpatient or reablement wards to determine discharge readiness and physiotherapeutic interventions required.
  • Parallel planning.
  • Develop and monitor individualised exercise and rehabilitation plans.
  • Liaise with hospital, community, and multidisciplinary teams to coordinate care.
  • Support timely flow and discharge.
