This is an excellent opportunity for a skilled practitioner looking to work within a fast-paced hospital setting, supporting timely and safe discharges while making a real difference to the lives of local residents. Working as part of a multidisciplinary team, you will play a key role in facilitating effective discharge planning, undertaking Care Act assessments, mental capacity assessments, safeguarding enquiries, and coordinating support for adults with complex needs.
