We have an exciting opportunity for a Care Coordinator to join our domiciliary and extra care services across Ellesmere Port and Chester.
As a Care Coordinator, you'll play a vital role in ensuring the smooth delivery of high-quality, person-centred care. You'll work closely with care staff, the people we support, families and healthcare professionals to ensure services are safe, effective and tailored to individual needs. This is primarily an office-based position with occasional field support when required. You'll work on a structured shift rota covering 7am-3pm and 3pm-11pm shifts, including alternate weekends and on-call duties.

- Plan, organise and manage staff rotas to ensure support visits are delivered efficiently
- Review and update schedules to meet changing service requirements
- Complete assessments and care plans to maintain excellent standards of care

The Person:
- Previous experience in care coordination or scheduling
- Strong organisational and communication skills
- The ability to manage multiple priorities effectively
- Experience maintaining accurate records and documentation
- A commitment to delivering outstanding person-centred care
- A full UK driving licence and access to your own vehicle
- A Level 3 qualification in Health and Social Care would be advantageous.
