Summary

We are looking to appoint a Social Worker to our Adult Community Mental Health Services Team in North Hertfordshire.

Transport links to medieval Hertfordshire and its lovely countryside are good, and abut London and beyond.

We are seeking a committed Social Worker to provide care for persons with mental health challenges.

  • Main responsibilities
  • Champion social care, having a good awareness of legislation and statutory advice.
  • To be the lead in the promotion of adult safeguarding practice, doing safeguarding enquiries and contributing to the development of care plans to protect adults and children from abuse.
  • To undertake full assessments and reviews for people who may present with high risk, or those with significant interpersonal, social, legal and/or environmental complexities, ensuring assessment against the Social Care Eligibility Criteria as defined by the Care Act 2014.
  • To provide information and advice to service users and carers and to colleagues to ensure those who do not fulfil the criteria for social care eligibility are effectively signposted and supported.
  • To ensure that appropriate health and social care services are commissioned with and for service users (including the obligation to provide direct payments) and that arrangements are in place for the local authority to invoice service users for the social care services provided, where appropriate.
  • Commissioned services should be enabling to service users and carers and targeted at persons living in the community with appropriate support and access to universal services.
  • Regular risk assessments must be carried out to protect the safety of vulnerable people.
  • To carry out assessments of carers, including identification of contingency plans and provision of support.
  • Experience in working in partnership to discover, develop and facilitate a wide range of recovery-focused community services and to keep colleagues within the locality aware of advancements and best practice.
  • To involve service users and their carers so the Trust can learn from their experiences and improve service delivery.
  • To maintain up-to-date electronic records of all work carried out.
  • To undertake post‑qualifying training and any necessary training to guarantee high standards of practice and continuance of registration with Social Work England.
  • To offer management and reflective supervision to freshly qualified social workers and to unregistered social care staff.
  • To support freshly qualified social workers through the Assessed and Supported Year in Employment (ASYE), including learning as an ASYE assessor.
  • To be willing to conduct duties as an Approved Mental Health Professional (AMHP) or to pursue AMHP training.
  • To conduct regular supervision and regularly reflect on own practice as a social worker and AMHP.
  • To conduct various tasks and duties which form part of the delegated social care responsibilities assumed by the Trust on behalf of Hertfordshire County Council.
  • Follow Trust policies and procedures.
  • To complete any additional training needed.
  • Perform any other duties reasonably determined by your Line Manager.

Qualifications

  • Registered with Social Work England.
  • Experience in working in a community context, providing service to a designated caseload of clients in the community who are experiencing or recovering from mental health challenges.
  • Prepared to pursue AMHP training if not already an AMHP; existing AMHPs will be on the Trust’s countywide AMHP rota.
