This is an exciting opportunity to join the nursing team at Eastwick Park Medical Practice in a part time position. The post has arisen as we are looking to enhamce the nursing team we have.
We are looking for a Health Care Assistant who is able to provide a high standard of patient care and services such as wound management, long term condition management, and cervical smears. The role is varied and covers a range of duties, and training can be given if needed to the right candidate. We are a supportive team and actively encourage and support learning and career progression. We believe in teamwork and we are looking for the correct candidates who understand this and will fit into our team ethos.The successful candidate must possess the ability to work within the MDT as well as being able to work independently and manage their own caseload.
Hours are negotiable to suit business needs and patient care delivery. Salary scale is dependent upon experience and qualifications. You will entitled to 5 weeks holiday + bank holidays pro rata to hours worked.
Salary is dependent upon skills and experience. There is the option to join or remain part of the NHS Pension scheme.
Annual mandatory training as well as paid training courses depending upon business needs.
The successful candidate will undergo a DBS check which the surgery will pay for.

Job Summary:
- To provide care for the patient including history taking, clinical assessment, diagnosis, treatment and evaluation of care.
- Be able to work both autonomously and collaboratively within both the clinical and administrative teams.
Key Responsibilities.
Assess, diagnose, plan, implement and evaluate treatment for patients, whilst promoting evidence based care and prioritising problems appropriately.
Provide nursing treatments, general and specific health screening / long term condition management to patients.
- Advise, support and administer vaccinations and injections for patients.
- Undertake sample collection including venepuncture and swabs.
- Utilise and demonstrate effective communication styles, to ensure patients are fully informed and consent to treatment.
- Act as an advocate for patients and colleagues.
Recognise and work within own competence and professional code of conduct as regulated by the NMC.
- Produce accurate, contemporaneous and complete records for patient consultations, consistent with legislation, policies and procedures.
- Prioritise and manage workloads in a manner that maintains and promotes quality.

About us
Eastwick Park Medical Practice is a well established pratice in Bookham, Surrey We provide care to a practice population of approximately 8400 patients. The wider practice team consists of 20 clincians who work part time both at the practice and remotely and 12 member sof the Patients Services Team. We are a training practice for GP\'s and Nurses.
We provide a supportive and caring team environment which radiates throughout the whole practice team. We believe in working hard to meet the needs of our patient population, and like to go the extra mile to achieve this where possible
Job responsibilities
The following are the core responsibilities ofthe Healthcare Assistant in delivering health services. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels:
- Consult with patients and their families toundertake assessments of patient need
- Deliver and assist clinical staff in the provision oftreatment, preventative care, health promotion, long-term conditionmanagement and patient education
- Be an integral part of the general practice team
- Provide clinical support to registered nurses andother healthcare professionals within the organisation
- Deliver basic clinical care interventions undersupervision, supporting continuity of care and patient safety initiatives
- Act as a chaperone as required
- Ensure specimens are recorded and ready foronward transportation
- Provide support during minor operations asrequired
- Ensure clinical waste is removed from clinicalareas and sharps bins replaced in accordance with the organisations IPC Policy
- Work within national and local protocols wherethese exist
- Be aware of your duties and responsibilitiesregarding current legislation and adhere to practice policies and procedureson Safeguarding Adults and Safeguarding Children
- Recognise the boundaries of their practice andknow when and to whom patients should be referred
- Support in the delivery of enhanced services andother service requirements
- Support the administrative team on anas-required basis
- Undertake all mandatory training and inductionprogrammes
- Contribute to and embrace the spectrum ofclinical governance
- Maintain a clean, tidy, effective working area atall times
- Attend a formal appraisal with their manager atleast every 12 months. Once a performance/training objective has been set,progress will be reviewed on a regular basis so that new objectives can beagreed

- Participate in any audits as directed
- Participate in local initiatives to enhanceservice delivery and patient care
- Support and participate in shared learning
- Complete opening and closing procedures inaccordance with the duty rota
