To start asap.An exciting opportunity has arisen within Adult Social Work Services for a motivated and compassionate Community Care Worker to join our newly established Prevention and Early Intervention multi-disciplinary team. Based at East Lothian Community Hospital, this innovative team works collaboratively with our NHS partners and those from Community First, hospital flow and discharge services, reablement, social work and care-at-home teams.The team’s core focus is to prevent unnecessary hospital admissions and support timely, safe discharges—reducing the impact of unscheduled care and promoting recovery at home. The successful candidate will adopt an “In Reach” approach within Edinburgh hospitals and carry out assessments for individuals in the community, ensuring responsive and person-centred support.This post currently operates Monday to Friday, with the potential for future seven-day working. It is anticipated that weekend working will be required up to two weekends per month at a maximum.While eligible for the Council’s flexible working scheme, the nature of this role demands a high level of agility and responsiveness.
