A Care Navigator is a very special person! They are the first port of call for patients and they respond to requests for GP appointments, made by patients either on the phone or in person. With training and support, they will triage the patient non-clinically, and signpost to the appropriate service or person, which may not necessarily be the GP. They will work in a reception hub alongside their colleagues (a team of 9) or on front desk reception.

We are looking for someone who is flexible enough to cover early starts (8am) and late finishes (6.30pm) across the week over 24 to 28 hours including a Monday, Tuesday and Thursday at least.

Main duties of the job

This is a challenging but very satisfying role. Patients rely on our care navigators to listen, advise and point them in the right direction. Often, patients themselves do not know what they need in terms of their health and it is up to a trained navigator to support them. Our team prides itself on the care and customer service we offer. Our access to appointments is excellent and prompt, but we rely on our navigators to ensure that the right person is seen at the right time by the right clinician (not necessarily the doctor!). This takes tact and diplomacy as well as patience.

About us

East Trees Health Centre is based in inner city Bristol. We are a training practice with a list size of 15,700 patients. Our practice team comprises of 4 partners, 7 salaried GPs, a paramedic, nurse practitioner, physio, podiatrist, dietitians, care co-ordinator, care navigators, administrators, nurses and pharmacy team. We are a very friendly, close-knit team and have regular staff and team meetings to ensure communication is consistent.

Job responsibilities
  • Support the GPs and team.
  • Signpost patients to appropriate services or clinician, by triaging non-clinically.
  • Book and cancel appointments.
  • Work with outside agencies to support the patient.
  • Deal with requests from patients for private income letters.
  • Meet and greet patients on front desk, and book in patients from other practices who are accessing appointments with midwife, DVT, Urology and other services.
  • Manage e-consultations in a timely fashion.
Person Specification
  • Experience in working with people.
  • Experience of working in a health care setting.
  • Ability to work by themselves as well as within a team.
