A rare and exciting opportunity has arisen in the Joint Community Rehabilitation (JCR) Team! If you have strong organisational, verbal and written skills and would enjoy helping your team, then this could be the role for you.
The JCR team help people to:
- remain in their own home and maximise their independence
- avoid going into hospital unnecessarily
- become independent as soon as possible after a stay in hospital
You will be working in partnership with the Duty Coordinators, Team Leaders and Management Team Countywide to coordinate the team of Support Workers as they provide support to people in their own homes. You will use an electronic rostering system to allocate visits to Support Workers, taking into account peoples needs and preferences whilst efficiently utilising the staffing resources available.

- have minute taking skills or experience
- are flexibility with working hours
