Are you energetic, self-motivated and committed to working across agencies, as well as strengthening partnerships, particularly with health? If so, then we would like to hear from you. An exciting opportunity has arisen to join the ASC Hospital Service as an Operations Manager, overseeing the Hospital Community Discharge Teams and the Continuing Health Care Team. You will play a pivotal role in managing community hospital inpatient assessments and discharges, as well as facilitating smooth transitions through community health pathways.

You will manage a team of Practice Managers and receive regular individualised support and supervision to carry out this role. Alongside Head of Service, you will help to continue new developments and changes to support the everyday processes within this service. You will need to be able to demonstrate the knowledge and experience to support the following key responsibilities required of this role.

The role involves leading and managing the ASC Continuing Health Care Team alongside two Community Hospital Discharge Teams, ensuring all practice complies with statutory duties under the Care Act, including assessments, care plan reviews, and safeguarding responsibilities. It includes chairing complex multi-agency meetings such as Adult Safeguarding Enquiries and Best Interest Meetings under the Mental Capacity Act, while also working closely with the Service Management Team to oversee cases involving Court of Protection applications and Coroner’s Inquests.

We are looking for candidates who can:

  • Translate local and national policy into practice
  • Contribute to and implement new service initiatives
  • Manage budgets and display effective leadership skills
  • Interpret legislation in difficult cases
  • Display working knowledge of the Capacity Act (2005) and DoLs
