The job itself Futures+ is one of the adult specialist services within East Riding of Yorkshire Council. We work with people with learning disabilities and/or autism who have high acuity and complex needs. Our team is composed of two hubs: The Preparing for Adulthood (PfA) Hub, and The Community Inclusion Hub. Together, we work closely with a dedicated team of Enabling and Transitioning Officers, Independence and Wellbeing Officers, Technical Assistants, an Autism Engagement Officer, and a Co-production Lead. Our main focus is to assess individuals’ care and support needs and co-produce packages of care and support that promote independence, wellbeing, and inclusion.

About the role: We're looking for an experienced Social Work Practitioner to join our team. You are expected to work with some of the most complex cases. A few days each month, you'll act as a duty officer working with another team member, handling enquiries and urgent requests from the public and professionals. You are expected to come into the office when you are on duty. You are expected to undertake Section 42 enquiries under the Care Act 2014, following the principles of Making Safeguarding Personal. You are expected to provide supervision and support to Enabling and Transitioning Officers as part of your role. As an advanced social worker, you are expected to step up to support our senior social workers in all areas of related tasks. Occasionally, in accordance with the service needs, you may need to work outside office hours.
What we are looking for: You must be a registered Social Worker with:
- Strong legal literacy and knowledge of the Care Act 2014, Children and Families Act 2014, Mental Capacity Act 2005, Human Rights Act 1998, and Equality Act 2010.
- Experience of Care Act assessments, MCA & best interest, Com DOLs, CHC checklist with strong analytical skills and report writing skills.
- A sound understanding of person-centred and strengths-based approaches.
- Proven experience in multi-agency working and partnership collaboration.
- Confident, competent, assertive with excellent interpersonal skills.
