Are you passionate about making a difference for vulnerable adults in East Riding? We are seeking a dynamic and organised individual to join our expanding Brokerage Team. You will play a vital role in sourcing residential and nursing care placements for adults including those with complex mental health needs and individuals requiring end-of-life care.
Key Responsibilities
- Build and Sustain Relationships: Proactively liaise with external organisations, partners, clients, and their families, establishing strong rapport and maintaining effective working relationships.
- Information Management: Gather, analyse, and accurately record essential information using a range of IT systems, ensuring data integrity and confidentiality.
- Collaborative Communication: Communicate clearly and effectively with colleagues across the Brokerage Team and other departments to ensure seamless service delivery.
- Problem Solving & Prioritisation: Demonstrate strong problem-solving skills, with the ability to plan, prioritise, and manage competing demands in a fast-paced environment.
