The East Riding of Yorkshire Council's Adult Social Care Service has a great opportunity to work with a dynamic team in delivering care and support to vulnerable adults. We need a suitably qualified, energetic and resilient individual to take up this exciting opportunity and help us to achieve our aims and offer a quality service to adults with social care needs.

We are seeking to recruit for a full-time care co‐ordinator post in the Beverley Community Wellbeing Team on a permanent basis. Beverley Community Wellbeing team is a busy team dealing with a high number of case load management.

You will be responsible for the delivery of care to vulnerable adults, providing assessments under The Care Act and the reviews of these assessments in the community. You will be supporting adults to maintain independence using the skills and local knowledge to identify support in the community. This will include a strengths based approach working style to maximise independence and self‐management.

You will be working in the Beverley area but be expected to cover at times areas within East Riding so must have access to transport.

The role of the care co‐ordinator is to gather information in order to determine the most appropriate response to people. This will range from people whose situation demands a crisis same day response within the duty through to the provision of information, assessment, support planning, financial advice, reviewing of care packages and multi-disciplinary working.

The work, although has an office base, is predominantly out in the field in the community, residential homes, hospitals, other placements and independent homes. The role is demanding and pressurised. The post-holder will be required to have knowledge of the policies and procedures that apply to adult care management and Adult Social care legislation. The post holder can expect regular professional supervision and access to quality training.

A confident, courteous telephone manner is essential along with the ability to competently use IT systems. All work undertaken is recorded on to the Adult Social care database. Ability to use Microsoft applications is an advantage.
