An exciting opportunity has arisen to join our Home from Hospital team, which works across East Renfrewshire supporting the safe and timely discharge of people from hospital.
You will have a good understanding of health and social care; the positive impact of timely discharge from acute settings; the value of outcomes focussed supports for people to enable them to return to their communities and recognise the value of forming effective relationships with other professionals and agencies.
We are a modern and ambitious Health and Social Care Partnership who are focussed on the people of East Renfrewshire achieving their full potential, regardless of any additional barriers.
This post is considered Regulated Work with Children and Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007.
