The predicted salary is between 25000 - 30000 £ per year.

An exciting opportunity has arisen to join our Intensive Services team, supporting the delivery of person centred care services across East Renfrewshire and enabling individuals to remain safe and supported within their own home.

You will have a good understanding of health and social care, the positive impact of timely discharge from acute settings and the value of outcomes focussed supports for people to enable them to return to and remain in their communities. You will also recognise the value of building and maintaining effective relationships with other health and care professionals and agencies.

You will be involved in activities such as undertaking assessment and review of service provision to service users, supporting the referral pathway for access to care at home services from hospital and community, and assisting individuals to continue living in their own homes by arranging care services and providing appropriate advice and support.

This post is considered Regulated Work with Children and Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007.
