An exciting opportunity has arisen to join Derbyshire County Council’s Public Health Department, within Live Life Better Derbyshire, to support the work of the Health Improvement Team. Live Life Better Derbyshire (LLBD) supports people to identify their holistic health and wellbeing needs, and provides weight management, smoking cessation, and physical activity services across Derbyshire. Additionally, LLBD also provides Health Check, Hypertension, National Child Measurement and Healthier Futures services.
We have a wonderful opportunity to recruit a new Health Improvement Worker to join the LLBD Hub Team. The LLBD Hub is the first point of contact for all referrals into the service, as well as general enquiries from clients, referrers, partners, and staff. Health Improvement Workers supporting the hub will contribute to the delivery of the LLBD service by supporting clients over the phone to identify and address their health and wellbeing needs, and by assisting with the various referral processing pathways within the hub team. In addition, the post holder will also support with a range of other duties to ensure that clients are fully supported in a timely and effective manner.
Responsibilities may include (not exhaustive):
- Take incoming calls to support clients to identify and address their health and wellbeing needs through conducting a holistic wellbeing assessment and signposting to weight management, physical activity and smoking cessation support.
- Make outgoing telephone calls to clients to support and book them onto a range of LLBD services.
- Respond to calls, emails and text message queries to the service, being the first point of contact for a client.
- Support clients by signposting to services that will help them to make lifestyle changes.
- Use behaviour change techniques and motivational interviewing to support clients to address the priority needs identified from their wellbeing assessment.
- Support with the provision of Nicotine Replacement Therapies to Smoking Cessation clients.
- Following up with existing and previous service users.
- Providing ad hoc support for the Exercise by Referral Program as required, and other partner schemes as needed.
- Supporting with the distribution of resources to a range of stakeholders.
- Supporting with stock taking and monitoring of resources.
These posts will be based within the LLBD Hub Team located in Matlock. Working within a busy office, the successful candidate will require a good level of IT literacy including proficient use of Microsoft Teams, Word, Excel, and Outlook along with the ability to adapt to bespoke databases. The ability to use Microsoft Office 365 and its associated applications would be an advantage. It is also essential that the successful candidate has excellent communication skills with the ability to communicate with a wide range of clients being aware of barriers that may prevent a lifestyle change. Due to the nature of the service the successful candidate must demonstrate the ability and commitment to undertaking a comprehensive range of duties as well as demonstrating flexibility in order to meet the demands of the service. There may at times be a need to work out in the Derbyshire community.
