Derbyshire County Council is seeking to recruit a Health Improvement Worker to join the Community Health Improvement team within Amber Valley. The successful candidate will support clients to complete our Health and Wellbeing assessment & Body MOT’s at a range of settings across the community. You will contribute to the health improvement of the population by supporting and signposting people to local organisations and services.

Strong partnership working skills will be required to ensure LLBD is embedded in the local community and will involve attending community events and carrying out engagement work. You will work to support the NHS Health checks programme and CVD case finding initiatives.

An excellent level of IT literacy including proficient use of Microsoft Word, Excel, Outlook and 365, along with the ability to adapt to bespoke databases is a key requirement of this role. It is also essential that the successful candidates have excellent communication skills with the ability to communicate with a wide range of people.

Awareness of barriers that may prevent a client accessing essential support or making a lifestyle change is key. All successful candidates must demonstrate the ability and commitment to undertaking a comprehensive range of duties as well as demonstrating flexibility in order to meet the demands of the service.

As the role involves dealing with sensitive information and vulnerable individuals, it is essential that the successful candidates have a knowledge of confidentiality and GDPR.
