Enablement Support Worker – Adult Social Care (Short-Term Reablement)

Location: Derbyshire Dales

Contract Options: 18 or 24 hours per week (with flexibility for extra hours)

Pay: Competitive (Grade 5)

Shifts: Rota-based including days, nights, weekends & bank holidays

What You’ll Do

  • Support clients in their own homes, helping them regain independence and confidence.
  • Deliver personalised care plans including personal care, medication, domestic tasks, and wellbeing support.
  • Collaborate with a multi-professional team and contribute to multi-disciplinary discussions.
  • Accurately record progress and escalate concerns when needed.
  • Receive ongoing support and training to help you excel and grow in your career.
