An exciting opportunity has arisen within the Adult Social Care and Health Brokerage Service for a Broker specialising in Pathway 3 (P3) hospital discharge referrals from the Royal Derby Hospital. This is a key role in supporting timely, safe discharge for individuals with complex needs who require 24-hour bed-based care on leaving hospital. Pathway 3 supports people who have experienced a significant life-changing event and who are likely to require long-term residential or nursing care.

Working closely with Health and Adult Social Care colleagues, you will help secure appropriate placements in community-based residential and nursing care settings and contribute to Derbyshire’s Discharge to Assess (D2A) pathway. The role will also involve working together with a team of Brokers with scope for these roles undertaking P3 discharges from the Chesterfield Royal as well as wider Brokering and Duty across the service.

About the Role

  • Source and negotiate short- and long-term residential and nursing home placements for patients identified as requiring Pathway 3 and P2 support.
  • Liaise with care providers, families, hospital teams and social care practitioners to secure suitable, safe and timely discharges.
  • Provide clear and accurate information to partners regarding placement availability, costs, outcomes and service capacity.
  • Maintain detailed knowledge of local care markets, service models, availability and pricing.
  • Undertake some home care sourcing, arranging care packages for people in the community as part of general Brokerage duties.
  • Keep accurate records, prepare brief reports and handle information in accordance with data protection requirements.
  • Use a range of IT systems confidently to track referrals, monitor progress and report outcomes.
