Overview

Health Care Assistant – Would you be interested in a career within the NHS as a Health Care Assistant in the community? We are looking for a dynamic, motivated and enthusiastic person who has excellent communication, IT literate and problem solving skills alongside a passion for patient centered care to support people over the age of 18 living in Bedfordshire.

You will be part of a multidisciplinary team (MDT) with clinical and managerial support, and you will work within a wider MDT aligned to social care professionals in reflection of the NHS 10 year plan.

Responsibilities

  • Deliver quality care to a designated caseload of people in their home, following a care plan devised by the community nursing staff you will be working alongside.
  • Provide concise information in a daily handover to ensure changing needs are met and risks are escalated.
  • Be flexible and assist colleagues to cover service needs, including working across the wider integrated team.

Location and Schedule

Location: Mid Bedfordshire (Ampthill, Shefford/Biggleswade) and surrounding areas. The Biggleswade community nursing team serves Biggleswade, Sandy, Potton and surrounding villages; the Ampthill Community nursing team serves Flitwick, Barton and Woburn and surrounding Mid West Beds villages.

The Community Nursing Service operates between 08:00 and 21:00 hours with a variety of rotational shifts, including weekends, evenings and Bank Holidays. Part-time posts would be considered.

  • Access to a car is essential to carry out service duties.

We have posts available to work alongside the Biggleswade and Ampthill community nursing teams delivering care to people in the mentioned areas.

Location details and working pattern may include a variety of rotational shifts; all changes will be discussed at interview.
