Overview
You will be part of a MDT team in which you will receive clinical and managerial support, which is also part of a wider MDT team that is aligned to social care professionals in reflection of the NHS 10 year plan.
Responsibilities
- You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff with whom you will be working alongside.
- You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.
