The East Lancashire Alliance is seeking a Cancer Care Navigator to support the Early Detection & Prevention of Cancer across the Hyndburn Central PCN.
You will work within a multidisciplinary PCN team to ensure the early cancer journey is seamless for patients and to help deliver the PCN's aims for patient-centred care. The role involves coordinating referrals, tracking screening uptake, and supporting teams across GP practices, pharmacists and social prescribers.
