East Coast Community Healthcare are currently looking to recruit an additional enthusiastic and motivated senior nurse to provide shared leadership to a team of community nursing staff providing care overnight to patients registered within the Great Yarmouth and Waveney area.

The successful applicant will be an experienced registered nurse, with significant community experience and a passion for delivering high standards of clinical care. The service responds to urgent and unplanned care needs out of hours, including people who have fallen. The successful applicant will be skilled in providing advice to patients remotely where appropriate and making clinical prioritisation decisions to support coordination of the team's face to face visits. The right candidate will have experience caring for palliative patients and their families, often in highly emotive circumstances. Competence in urethral and supra-pubic catheters is essential. The successful candidate will be able to demonstrate how they embody our values, and a desire for personal and professional development.

Knowledge of local and national initiatives and evidence of lifelong learning and personal development are essential., The out of hours team form part of the Primary Care Homes 24/7 urgent response, which aims to support people at home and avoid unwarranted admissions. The PCH works closely with GP's and other members of the multidisciplinary team 24/7 to provide an excellent standard of person centered nursing care to adult patients in their own homes and residential care, supporting patients to self manage whenever possible. The senior nurse plays a key role in supporting the transition of care within 24/7 PCH care and/or partner organisations, ensuring unplanned needs are managed out of hours and an appropriate follow up plan is in place. In addition to clinical work, the senior nurse will have supervisory and line management responsibility, supporting the development of a highly experienced and well-embedded out of hours team. The senior nurse is accountable for the effective delivery of high standards of evidence based nursing care, including through the use of audits and supervision.

Applicants will be required to participate in working weekend and bank holidays on a rotational basis.
