An exciting opportunity has arisen for an enthusiastic and self-motivated Occupational Therapist to join our the Primary Care Home Hub (PCH) based at Hamilton house in Lowestoft. The Primary Care Home is a multidisciplinary team of Nurses, Therapists, Assistant Practitioners, Health Care Assistants, and Rehabilitation Support Workers delivering nursing, therapy and reablement/rehab services to adults in their own homes.

You will be completing occupational therapy assessments of patients with diverse presentations to provide a diagnosis of functional status to develop and deliver individualised person centred therapy plans.

You will hold responsibility for your own case load, provide support and encouragement to patients in order to promote independence. Respond to patient and family needs. Prevent unnecessary hospital admission and support end of life care at home.

You will have access to direct supervision. Supervision takes the form of individual and group meetings, clinical reasoning/Case study sessions and peer review.

Main duties of the job

To provide a high standard of occupational therapy within the integrated community services as part of a multi-disciplinary team to patients within a range of domiciliary settings.

To provide clinical case management for a caseload of patients who require occupational therapy.

Participate in rapid assessment for patients who present with a variety of acute, chronic, and age-related conditions.

To work autonomously at a level appropriate to clinical competence and within scope of professional practice.

To supervise delegation of work to the Therapy Assistant Practitioners. .

The service uses SystmOne as a clinical IT system to support mobile working.

The service operates 7 days a week 7:00 to 20:30 hours. Applicants for this post will be required to work between Monday to Friday, flexibility of start and finish times will be considered. At times you may be required to work with another of the 4 PCH\'s from a different location to cover staff sickness, or capacity pressures.

Lowestoft Primary Care Home is a multidisciplinary clinical team of health and social care professionals working in partnership with Primary Care, Social Services, social care providers, other specialist community health teams and voluntary sector providers to ensure patients are able to remain in their own homes whenever possible.

Job responsibilities

CLINICAL:

To be professionally and legally responsible andaccountable for all aspects of your own work including the management ofpatients in your care. To ensure a high standard of clinical care for thepatients under your management and support others to do likewise.

To interpret and analyse clinical and non-clinicalfacts to form an accurate diagnosis and prognosis in a wide range ofconditions, to recommend the best course of intervention and to developcomprehensive management strategies for the patient\'s presenting condition.

To undertake comprehensive assessment of patients,using investigative and analytical skills and to formulate individualisedmanagement and treatment plans, using clinical reasoning and utilising a widerange of treatment skills and options to formulate a specialised programme ofcare.

To accept clinical responsibility for a designatedcaseload of patients and to organise this effectively and efficiently withregard to clinical priorities and use of time.

To develop the skills and dexterity, co-ordination andpalpatory senses for assessment and manual treatment of patients. To be safeand competent in the use of therapy equipment, relevant to the rotationsetting, with support and training from the senior therapist as appropriate.

To recommend/ prescribe equipment and minoradaptations, following assessment and use of clinical reasoning skills, inorder to compensate for unresolved functional deficits, in line with localresources, guidelines and legislation.

To facilitate safe provision of equipment and instructpatients, family and carers in the use of equipment, as appropriate.

To assess, using clinical reasoning for minoradaptations, take measurements and communicate the recommendations, throughreports and diagrams to adaptation providers, with support from seniortherapists.

To have an awareness of potential psychological riskfactors and the functional implications of these.

To provide spontaneous and planned specialist advice,teaching, and instruction to relatives, carers and other professions to promoteunderstanding of the aims of occupational therapy and to ensure a consistentapproach to patient care.

To be responsible for equipment used in carrying outoccupational therapy duties and to adhere to any relevant policies, includingcompetence to use equipment and ensure safe use of equipment by others throughteaching, training and supervision of practice.

To supervise assistant practitioner and support staffwhen appropriate.

To provide clinical supervision for occupationaltherapy students on their first introductory placement and to explain the roleof occupational therapy to departmental visitors. Assist more senior staff inthe training of students and occupational therapy assistants.

To communicate effectively with patients and carers tomaximise rehabilitation potential and to ensure understanding of theircondition. Communication skills of persuasion, motivation and explanation willbe used with a variety of patients.

Communicate effectively with all members of theoccupational therapy team, AHPs and community nursing and other professions toensure a collaborative approach to the management of the patient\'s condition.

To assess capacity, gain valid informed consent andhave the ability to work within the legal framework with patients who lackcapacity to consent to treatment.

PROFESSIONAL:

To maintain own clinical professional development(CPD) by keeping abreast of any new trends and developments and incorporatethem as necessary into your work.

To be an active member of the in-service trainingprogramme by attendance at training sessions in-house, Tutorials and byattending external courses and practicing reflective practice.

To maintain accurate, comprehensive and up to datedocumentation in line with legal, departmental and ECCH requirements,communicating assessment and treatment results to the appropriate disciplinesin the form of reports and letters.

To participate in staff appraisal scheme and personaldevelopment plan (PDP).

To work within Organisational and Royal College ofOccupational Therapists OT guidelines and adhere to the Professional Code ofConduct.

To have a working knowledge of available equipment,minor adaptations and resources including Activities of Daily Living (ADL)equipment, therapy and rehabilitation aids, moving and handling equipment and abasic knowledge of wheelchair provision.

To undertake the measurement and evaluation of yourwork and current practices through the use of evidence-based practice (EBP)projects, audit and outcome measures, either individually or with otherclinical staff and the clinical lead for occupational therapy. Makerecommendations for change.

Demonstrate a sound understanding of ClinicalGovernance and Risk Management and apply to the work situation.

ORGANISATIONAL:

To be responsible for a designated area of work asagreed with the team leader and to plan and organize efficiently andeffectively with regard to patient management and use of time.

To decide priorities for a work area, balancing otherpatient related and professional demands. Ensure that these remain inaccordance with those of the occupational therapy service as a whole.
