Do you want to progress your nursing career with a supportive and encouraging team, providing nursing care to patients in their own homes, clinic or residential homes? The PCH is a multidisciplinary team delivering health care to people in Lowestoft. The team responds to the changing needs of patients and supports those who may previously have been admitted to hospital to remain at home. We would be happy to provide preceptorship to a newly qualified Registered Nurse.
The PCH team provides community services 7 days per week from 07:00 to 20:30. The core shift is 08:30 to 16:30 with additional shifts 07:00-15:00 and 12:00-20:00. Weekend and bank holiday work is on a rotational basis. The post is offered at 37.5 hours per week across 5 working days. Flexible working arrangements and other shift patterns can be considered.
Responsibilities
- Provide an excellent standard of person-centred care to adults in their own homes, residential care or clinic settings, including a mixture of nursing and reablement interventions to help patients maintain health or recover from ill health where possible.
- Complete holistic assessments for patients referred to the service.
- Participate in the delivery of nursing interventions which could include, but are not limited to:
- Holistic assessments including environmental, moving and handling assessments, pressure area risk, nutritional needs.
- Urinary catheterisation.
- Administration of medication via injection.
Qualifications and Requirements
- Registered Nurse (Adult) or a 3rd year student due to qualify.
- Willingness to provide high-quality patient care in community and clinic settings.
- Ability to work as part of a multidisciplinary team.

Additional Information
Lowestoft Primary Care Home is a multidisciplinary clinical team partnering with Primary Care, Social Services, social care providers, other specialist community health teams and voluntary sector providers to support patients to remain in their own homes whenever possible. The role is intended for early application as the job may close before the advertised date if sufficient applications are received.
