Responsibilities
- Communicate effectively with patients, relatives and carers, respecting patient confidentiality, privacy and diverse cultural backgrounds.
- Communicate complex, sensitive or contentious information to patients, relatives or carers, requiring persuasive, motivational, negotiating and empathetic skills.
- Act as a resource for community nursing colleagues and other health professionals regarding leg ulcer management across the Primary Care Networks.
- Participate in teaching and clinical supervision of support workers and other provider staff as required.
- Provide mentorship to pre‑ and post‑registration students.
- Contribute to the clinical governance agenda through participation in clinical risk assessment and management.
- Critically evaluate research findings, national guidelines and support the implementation of changes in clinical practice as appropriate.
- Participate in clinical trials as required.
- Make a positive contribution to excellent communications within the organisation.
- Contribute to the delivery of the Lower Limb Pathway, providing management of lower leg ulceration in a clinical setting within agreed time frames.
- Create an environment conducive to effective working, respecting and supporting staff to deliver high‑quality clinical services.
- Work with other members of the primary care home to deliver pro‑active services.
- Show flexibility to work across bases within the organisation if required.
- Work with colleagues to develop care pathways for patients with a range of healthcare needs.
- Undertake continuous professional development as identified through appraisal and personal development planning.
- Ensure compliance with the Nursing and Midwifery Code of Conduct.
- Ensure a high standard of record keeping in line with ECCH and professional standards.
- Report any concerns via the Datix incident reporting system.
- Take responsibility for the management of clinic resources, respecting people, time and money.
- Contribute to the ongoing review of the wound care formulary and guidelines.
