An exciting opportunity has arisen for a Care Coordinator to join our Primary Care Home (PCH) to deliver high quality care to the population of Gorleston and the wider Great Yarmouth and Waveney area. The role involves supporting the senior triaging clinician with delegated responsibilities, actively coordinating the multidisciplinary management of patients within the local community, and ensuring referrals and visits are managed in a timely manner, including urgent community responses.
The PCH is a multidisciplinary team working closely with GPs and other community partners to provide an excellent standard of person-centred care to adults in their own homes, residential care, or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working. The post is offered at 30 hours per week and sits in a multidisciplinary team providing community services 7 days per week from 07:00 to 20:00 hours.
As a Care Coordinator, you will be based at Yare House within Gorleston PCH. You will work in partnership with other members of the multidisciplinary team to coordinate and triage planned and reactive care within the community services. The post holder is required to work autonomously at a level appropriate to clinical competence and within the scope of professional practice. The post holder will work under the supervision and in partnership with registered staff, providing and supporting the delivery of high standards of care, contributing to the ongoing assessment, provision, and monitoring of care based on evidence and working in partnership with the multidisciplinary team.
