Responsibilities:

  • Promote recovery and well-being across a variety of clinical areas, empowering service users to self-manage and maximise independence using health coaching techniques and meaningful activity centred around individual goals.
  • Work autonomously at the level appropriate to clinical competencies and within scope of practice, combining assessment, intervention and ongoing referrals.
  • Provide training sessions for staff, families, carers and external agencies as required.
  • Collaborate with members of the multidisciplinary team to provide proactive and reactive services.
  • Contribute to the continuous evaluation of the service to ensure it is fit for purpose and increases choice through audit and research.

Role Details:

The successful individual will be based in Gorleston and work throughout the Great Yarmouth & Waveney area. They will join an enthusiastic and supportive multidisciplinary neurology team in providing high-quality care and rehabilitation. The role requires experience in supporting people with progressive neurological conditions and offers a strong commitment to clinical supervision, continuing professional development and opportunities for supervision, training and peer support. Collaboration will be close with Stroke Early Supported Discharge and primary care home colleagues.
