About the Role

We are looking for caring and compassionate Healthcare Assistants to become part of our Primary Care Home (PCH) team to deliver clinical interventions for patients with a wide range of health care needs in a variety of community settings.

  • Wound care
  • Venepuncture
  • Catheter maintenance
  • Promoting self-care
  • Administration of insulin to support our diabetic patients
  • Palliative care
  • End of life care
  • Rehabilitation support

The post holder will visit patients in their own homes or residential care to deliver these interventions either on their own or with another clinical member of staff. We will provide support and education to develop the required skills. We have a robust lone worker policy ensuring your safety whilst working within the community.

Hours and Schedule

This post is 22.5 hours. Shift patterns can vary across the core hours of 08:30 - 16:30, Monday to Sunday. Applicants will be required to work weekend and bank holiday shifts, working on a rotational basis.
