An exciting nursing opportunity has arisen for the right candidate to work in the Integrated Community Neurology Team as a Neurology Nurse, at East Coast Community Healthcare in the Great Yarmouth and Waveney area.
We are looking for an enthusiastic forward-thinking clinician with a strong interest in Neurology and experience in managing patients in the community.
Following a comprehensive training programme, we require you to work autonomously, be confident in managing patients independently, and possess good integrated working skills with the wider MDT.
The Integrated Community Neurology Team aims to provide excellent clinical management and community rehabilitation to people with Parkinson's Disease, Motor Neurone Disease and Multiple Sclerosis.
We work to deliver multidisciplinary, evidence-based practice that improves outcomes for people with long-term neurological conditions.
The Community Neurology Nurse will provide specialist skills and knowledge to patients, carers, professional and non-professional staff in Great Yarmouth and Waveney ; to support the already existing Neurology service in its development and management to work towards a seamless integrated neurology service., The post holder will work autonomously and have responsibility for the assessment, development, implementation and evaluation of planned programs of care in conjunction with the multi-disciplinary team for patients with the neurological conditions of Parkinson's Disease/ Multiple Sclerosis/ Motor Neurone Disease, within the community setting; in accordance with evidence based practice and local policies/ protocols/ guidelines.
- Supporting the Neurology Nurse Practitioner for Neurology Services in the implementation and day to day management of clinics, home visits, and acting accordingly to urgent reviews when required; working alongside Neurologists / General Practitioners on medication / therapy outcomes.
- Providing education and monitoring of injectable / oral disease modifying therapies in specialised clinics offering regular review and support.
Liaising with the Neurology Nurse Practitioner on the ongoing development of the Disease Modifying Therapy pathways in conjunction with secondary care.
Supporting MS patients through their Disease Modifying Therapy journey.
- Provide advice, information, support and clinical practice to newly diagnosed and existing neurological patients and their carers in primary, secondary and tertiary care.
The integrated Community Neurology Team consists of Nurses, Physiotherapists, Occupational Therapists, Assistant Practitioners and Support staff.
We work closely with colleagues in our Early Supported Discharge team and the wider community services in East Coast Community healthcare.
