An exciting opportunity has arisen for a compassionate and motivated Registered Nursing Associate (RNA) to join the Frailty and Community Outreach Team within the Primary Care Network.

Working as part of an integrated multidisciplinary team, the Nursing Associate will deliver high-quality, evidence-based nursing care to adults living with frailty, multiple long-term conditions and complex health needs across community and primary care settings.

Main duties of the job

The Nursing Associate will work as part of the PCN multidisciplinary team, providing person-centred care to adults with long-term conditions, frailty and complex health and social care needs.

The role includes completing delegated assessments and reviews, delivering planned clinical interventions, undertaking home visits and supporting patients to maintain their independence and manage their health.

The postholder will carry out a range of clinical skills, including physiological observations, venepuncture, diabetic foot checks, wound care and point-of-care testing, in line with training and competencies.

They will work closely with GPs, nurses, allied health professionals, community services and social care colleagues, contributing to care planning, multidisciplinary meetings and coordinated care.

The role will also involve recognising deterioration or risk, escalating concerns appropriately and supporting health promotion, medicines optimisation and admission avoidance.

East & Central Brighton Primary Care Network brings together nine GP practices, supporting a population of around 75,000 patients.

We have developed a broad multidisciplinary Outreach Team spanning occupational therapy, physiotherapy, social prescribing, paramedicine, health coaching, nursing, mental health and pharmacy.

• Provide compassionate, person-centred nursing care for adults with complex physical, mental health and social care needs.
• Undertake delegated nursing assessments within scope of practice, contributing to the development, implementation and evaluation of individualised care plans.

Person Specification

• Other essential Requirements
• Full UK driving licence.
• Access to a suitable vehicle.
• Ability to travel across the PCN.

Qualifications

• Registered Nursing Associate with current NMC registration.
• Foundation Degree Nursing Associate or equivalent.
• Evidence of continuing professional development.
• Qualified in Phlebotomy

Experience

• Experience as a Registered Nursing Associate in primary care, community or integrated care.
• Experience supporting adults with frailty and multiple long-term conditions.
• Experience contributing to nursing assessments and care planning.
• Experience of multidisciplinary working and community visiting.
• Frailty, long-term condition management, safeguarding, infection prevention, pressure ulcer prevention, falls prevention, Mental Capacity Act, consent and professional accountability.
• Venepuncture, clinical observations, NEWS2, urinalysis, wound care, pressure area assessment, patient education, documentation, communication, time management and risk assessment.
• Experience in a Primary Care setting.
