NHS GP Practice Nurse in London

Earls Court Surgery is a friendly, community-focused GP practice in the heart of Earls Court, providing high-quality primary care to approximately 4,000 patients. Our clinical team includes four GP partners, three part-time salaried GPs, a Healthcare Assistant, and an administrative team who work closely together to deliver excellent patient care.

We are looking for a compassionate and enthusiastic part-time Practice Nurse to join our supportive team. This is an excellent opportunity to work in a well-established practice where teamwork, continuity of care, and patient-centred service are at the heart of everything we do.

Main duties and responsibilities

• Provide routine nursing care, including wound care, dressings and cervical screening.
• Deliver NHS vaccination and immunisation programmes for adults and children.
• Support the management of patients with long-term conditions such as diabetes, asthma, COPD, hypertension, and cardiovascular disease through reviews and monitoring.
• Undertake health promotion activities, including NHS Health Checks, smoking cessation advice, weight management, and lifestyle counselling.
• Carry out venepuncture, ECGs, blood pressure monitoring, and other routine clinical procedures.
• Administer prescribed medications and injections in accordance with Patient Group Directions (PGDs) or practice protocols.
• Maintain accurate and timely clinical records using the practice's clinical system.
• Work collaboratively with GPs, Healthcare Assistants, and the wider multidisciplinary team to provide safe, effective, and coordinated patient care.
• Participate in infection prevention and control, safeguarding, quality improvement, and clinical audit activities.
• Contribute to the achievement of Quality and Outcomes Framework (QOF) and other contractual requirements.

Job responsibilities

• Responsible to: The GP Partners and Practice Manager
• Job Summary: The Practice Nurse will provide high-quality, evidence-based nursing care to patients registered with the practice. Working as part of the multidisciplinary team, the post holder will deliver a wide range of clinical services, promote health and wellbeing, support the management of long-term conditions, and contribute to the achievement of practice and NHS quality standards.

Main duties and responsibilities

• Deliver routine nursing care in accordance with evidence-based practice.
• Undertake cervical screening (where appropriately trained).
• Administer childhood and adult immunisations in line with national schedules.
• Administer seasonal vaccination programmes, including influenza and COVID-19 vaccinations where applicable.
• Administer prescribed medicines and injections under PGDs, PSDs, or independent prescribing qualification where appropriate.
• Perform venepuncture, blood pressure monitoring, ECGs, spirometry (where trained), and other clinical procedures.
• Carry out wound assessment, wound care, and dressing changes.
• Remove sutures and clips.
• Assist with minor surgery and other clinical procedures as required.
• Undertake NHS Health Checks and health screening programmes.
• Provide travel health advice and administer travel vaccinations (where appropriately trained).
• Offer lifestyle advice including smoking cessation, weight management, alcohol reduction, healthy eating, and physical activity.

Long-Term Condition Management

• Support the assessment, monitoring, and management of patients with long-term conditions including: Diabetes; Asthma; COPD; Hypertension; Cardiovascular disease; Chronic kidney disease.
• Work within agreed clinical protocols to optimise patient outcomes.
• Recognise deterioration and refer appropriately to GPs or other healthcare professionals.

Health Promotion and Prevention

• Promote health education and disease prevention.
• Support national screening programmes.
• Encourage patient self-management and shared decision-making.
• Identify patients requiring additional support and refer to appropriate services.

Patient Care

• Assess patient needs and provide holistic, compassionate care.
• Maintain patient dignity, privacy, and confidentiality.
• Respond appropriately to urgent clinical situations.
• Escalate safeguarding concerns in accordance with practice policy.
• Ensure patients receive appropriate follow-up and continuity of care.

Clinical Governance

• Maintain accurate, contemporaneous electronic patient records.
• Participate in clinical audit and quality improvement activities.
• Support the achievement of Quality and Outcomes Framework (QOF) and other contractual requirements.
• Maintain professional competence through continuing professional development.
• Participate in mandatory training and annual appraisal.
• Comply with infection prevention and control policies.
• Ensure clinical equipment is maintained, calibrated, and safely used.
• Maintain safe and well-stocked clinical areas.

Team Working

• Work collaboratively with GPs, Healthcare Assistants, pharmacists, social prescribers, and the wider multidisciplinary team.
• Provide support and mentorship to students or new staff where appropriate.
• Participate in practice meetings and contribute to service development.
• Demonstrate flexibility to meet the changing needs of the practice.

Professional Responsibilities

• Maintain current NMC registration.
• Work within the NMC Code of Conduct.
• Adhere to all practice policies and procedures.
• Maintain patient confidentiality and comply with UK GDPR and information governance requirements.
• Take responsibility for maintaining professional knowledge and clinical competence.
• Undertake any other duties appropriate to the role as reasonably requested by the GP Partners or Practice Manager.

Person Specification

• Competence in adult immunisations and vaccinations.
• Experience in wound care and dressings.
• Venepuncture and ECG competency.
• Experience in the management of long-term conditions such as diabetes, asthma, COPD and hypertension.
• Excellent communication and interpersonal skills.
• IT literacy and experience of using electronic patient record systems.
• Experience using SystmOne clinical system.
• Experience working towards Quality and Outcomes Framework (QOF) targets.

Qualifications

• Registered General Nurse (RGN) with current registration with the Nursing and Midwifery Council (NMC).
• Evidence of continuing professional development (CPD).
• Competence in cervical cytology (or willingness to undertake training).
• Previous experience working as a Practice Nurse within primary care.
