ABOUT THE ROLE

The Respond Healthcare employed Community Stoma Care Nurse (CSCN) will be dedicated to the development of the Stoma Care Nursing service across the Birmingham community and surrounding areas and will provide support to acute hospitals to generate referrals to the Respond service. The purpose of the Respond CSCN is to provide professional and ethical nursing care services to patients by working in partnership with key stakeholders. The CSCN will directly contribute to the growth and success of Respond Healthcare by developing an ethical Stoma Care Nursing Service within the community and supporting NHS acute services whereby community support is required.

Through this service the Nurse role will be to ease a smooth transition for patients from Hospital to the community, to support and manage the care of patients in the community following their discharge from hospital, and to perform home visits in line with agreed patient referral pathway models, and to work as an extension of the acute based Teams. The CSCN will be required to work autonomously in the Community and therefore must be self‑motivated with the drive and passion to develop their own Community Service whilst combining commercial awareness, expertise and advanced clinical judgement. The CSCN will be employed by Respond Healthcare Limited and report directly to the Nurse Team Leader.

As a Respond CSCN, the role always requires the delivery of professional and ethical care to patients. The role is to support new patients referred onto the Respond Dispensing Appliance Contractor (DAC) service from Hospitals across the Birmingham regions, and the surrounding areas where applicable. To develop your community service with NHS Trusts and community services and introduce the Respond DAC service to patients in the community. The Nurse will utilise commercial awareness in the use of the group medical appliances and ancillary products where suitable and appropriate, and always in line with clinical and ethical professional practices; ensuring patients clinical needs are being met.

KEY RESPONSIBILITIES

  • Working in collaboration with NHS Trust Stoma care Teams to support patient discharges onto Respond following their surgery relative to your speciality, i.e. bowel, bladder or stoma care.
  • Working as an extension of the acute stoma care team and support the introduction of the patient pathway of care within the community through patient home visits.
  • Recording of patient information on the Respond Consult patient management system.
  • To work alongside Nurse Team Colleagues.
  • Support the group's development of educational programmes within the HCP Network to ensure the Eakin Healthcare Group and its wholly owned subsidiaries are recognised as a supporter of Nurses medical education and continuous professional development.
  • Support the development of patient focus groups within your territory where possible – to review new product development ideas, support patients locally, enhance relationships and promote the service that the Respond Nurse and the service can provide.

Administrative

  • Introduce and maintain appropriate and efficient systems of referral and review systems of documentation via the Respond Consult Patient Management Systems.
  • Organise workload to take account of priorities of care in meeting the needs of a defined caseload.
  • Provide written reports as required to the Line Manager regarding issues relating to the provision of care, monthly reports and data collection, annual report and service audits.
  • Effective communication with colleagues in the Hospital, community, Respond Nursing Team and other agencies as appropriate.
  • Attend relevant nurse team meetings and discussions as required.
  • Provide a cost‑effective service.

Commercial Awareness

  • Perform Appliance Use Reviews (AURs) where appropriate – in line with company and Department of Health Guidelines.
  • Support the promotion of the Eakin Groups high quality, clinically effective stoma care products and accessories where appropriate, whilst maintaining clinical and ethical judgement.
  • Support the growth of the DAC by promoting the Respond Dispensing Service; whose objective it is to provide the patient with the best possible clinical outcomes and service.

Team Working

  • Work closely with the customer services team who will support the patients ongoing care through the Respond Dispensing appliance contractor service provision.
  • Develop strong working relationships with your key internal team members and colleagues across the Eakin Healthcare Group of companies.
  • Create relationships with key external stakeholder where applicable for your role.
  • Enjoy participating in group activities and comfortable supporting colleagues and fellow Healthcare professionals in the acute setting when needed and where SLAs and Honorary Contracts allow it.
  • Willing to co‑operate and compromise for the overall good of the patients care.
  • Actively participates in team building activities and internal Nurse meetings.

WHAT WE’RE LOOKING FOR

Essential

  • Must possess an appropriate Stoma Care Qualification or be willing to complete the appropriate courses / qualifications with existing experience in Stoma Care at a Nurse Associate Level.
  • Registration on Part 1 of the NMC Register.
  • Maintain and comply with the NMC code of professional conduct standards.
  • Minimum of twelve months post registration experience in Stoma Care Nursing.
  • Be able to provide evidence of Personal Professional Development relevant to the speciality.
  • If not already held, you must be prepared to complete the NMC Community Practitioner Nurse Prescribing Course (also known as v100 or v150 course) within an agreed time period, should the future enhancement of the service within your territory require it.
  • Membership of RCN or similar.
  • Excellent communication skills both verbal and written.
  • Demonstrate good IT proficiency.
  • Ability to work autonomously, dynamic and forward thinking.
  • Ability to work as part of a wider multidisciplinary team and support your fellow healthcare professionals.
  • Flexible approach to working.
  • Can work under pressure, remains calm, thinks clearly and manages emotion under pressure.
  • Caseload management including time management skills.
  • Be responsible for the development and implementation of your stoma care nursing service provision across the acute and community setting.
  • Maintain accurate clinical records in line with company, legal and professional requirements.
  • Ability to write concise reports and maintain accurate records.
  • Commercial awareness with the capability to drive patient referrals onto the Respond service.
  • Full and valid driving licence.

ADDITIONAL INFORMATION

This is a community‑based role. Your time will therefore be split between the community and the hospital setting where required. Setting up of clinics in the community if this is required. Your place of work will be in the Community, however as a Company Stoma Care Nurse, you may be required to travel within the UK to attend conferences, commercial and Nurse team meetings and any educational courses, events, study days etc as deemed necessary for your professional development. Occasional travel required to attend meetings and / or training days at any of the company offices. Your role may require you to support NHS Trust Stoma Care Departments by covering holiday and sick leave where Honorary Contracts or Service Level Agreements permit this. You will be required to spend nights away from the home occasionally during each financial year as required by the business. You will be required to attend the annual Association of Stoma Care Nurses (ASCN) UK educational conference. The Company is committed to supporting your continuous professional development and will fund future training and educational needs including but not limited to; ASCN, prescribing courses, degree courses where appropriate, and any formal qualifications not previously held and deemed necessary for your role.
