We are recruiting on behalf of our client for an experienced Advanced Adult Social Work Practitioner to join their Hospital Social Work Team. This role involves leading practice, supervising staff, and working closely with health partners to support safe and timely hospital discharges.
Responsibilities
- Lead and support the hospital social work team
- Supervise and manage social work staff
- Attend daily multidisciplinary meetings with health partners
- Support safe, timely discharges across hospital pathways
- Manage a small caseload alongside line management duties
- Complete Care Act, Mental Capacity, and Best Interest Assessments as required
Essential Requirements
- Qualified Social Worker (Social Work England registered)
- Proven adults social work hospital experience
- Experience supervising and managing social work staff
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act 1998
- Experience working with complex needs (e.g. substance misuse, homelessness, self-neglect)
- Confident decision-maker with strong risk management skills
- Good IT skills, including Liquid Logic and Excel
