An exciting opportunity has arisen for an Assessment and Support Coordinator within the Integrated Flow Team based at Dumfries and Galloway Royal Infirmary. This is a permanent post of 30 hours per week, 52.14 weeks per year.
Your primary responsibility will be to work in partnership with the person and formulate or help facilitate care provision to support hospital discharges. This will be in conjunction with service users’ families where appropriate, and alongside colleagues from the Hospital, Home Team, and other agencies and disciplines in the community.
You will need to be able to work under your own initiative and prioritise accordingly as well as being an integral member of a busy social work team. You will preferably have a knowledge base that encompasses aspects of social care for adults with priority needs, and their support plan solutions will involve a Personalised Approach in line with departmental policy, good practice and the legislative requirements of the Social Care (Self Directed Support) (Scotland) Act 2013.
You will also be expected to practice using protective legislation, in particular the Adult Support and Protection (Scotland) Act.
