The Learning Disability Service provided by Dumfries and Galloway’s Health & Social Care Partnership supports young people and adults who have a range of needs including learning/physical disabilities and autism spectrum conditions.

Castle Douglas Community Support Service provides both long-term care and support to adults living in their own homes and short-term Outreach support to families to prevent and respond to crisis situations. The service is expanding its Enhanced Outreach Service and is keen to hear from applicants who are adaptable and committed to supporting families at times when extra support is needed. As a Senior Support Coordinator, you will be part of a team committed to delivering high-quality person-centered services, putting Health & Social Care Standards at the heart of all we do. Reporting to the Manager, this will involve an element of direct care and support to people in their homes and in accessing community opportunities to develop skills, promote independence, improve health and wellbeing and participate in adult learning/work-related activities. You will be overseeing personal planning and reviews, team meetings and recording electronic information. To supervise a team of Key Workers/Link Workers to assess, develop, coordinate and implement support services for Day, Community and Home Support Service users appropriately to individual assessed needs and personal outcomes.

In this post you will be required to work 34 hours per week 52.14 weeks per year. Applicants should possess SVQ Level IV in Health and Social Care (or equivalent qualifications).

You must have some experience of paid or unpaid work with people with additional support needs, for example learning/physical disability, older adults or mental health conditions. You must be a team player, committed to supporting and enabling people to live fulfilled lives and to supporting service improvements. An ability to use basic computer applications is desirable but training can be given.

This post is considered as Regulated Work with Vulnerable Children and Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. From 28 February 2011, preferred candidates will be required to join the PVG Scheme or undergo a PVG scheme update check prior to formal offer of employment being made by Dumfries and Galloway Council. Applicants who have been resident outside the UK within the last 5 years, if offered a position, in addition to a Disclosure Scotland Check, will be required to provide a completed police record check from the police authority in the country or countries in which they were resident.
