Responsibilities
- Implement evidence‑based processes for managing patients with long‑term conditions, including care for elderly and housebound patients.
- Deliver nursing services as a senior member of the multidisciplinary team.
- Provide care in infection prevention and control, health promotion, chronic disease management, well‑woman and well‑man clinics.
- Support the practice management team in reviewing and delivering clinical policy and procedure.
Essential Qualifications
- Registered Nurse with the Nursing and Midwifery Council.
- Experience in chronic disease management (diabetes, hypertension, respiratory conditions).
- Previous practice nurse experience.
- Qualified smear taker.
- Immunisations/Vaccinations trained, including childhood immunisations.
- Clear DBS, right to work, etc.
Desirable
- Experience using EMIS.
- Diabetes qualification.
- Asthma/COPD qualification experience.
- Travel health knowledge.
- V300 prescriber.
