About the service:
Join a high-impact UCR/IFD team delivering rapid, high-quality community care to prevent unnecessary admissions and support NHS 4-hour and 12-hour ED standards.
Responsibilities:
- Lead delivery of the 2-hour UCR for adults (18+).
- Act as the lead GP across UCR and IFD, coordinating hospital and community pathways.
- Assess and manage crisis presentations at home and via ED referrals to avoid admission.
- Chair daily MDTs; set safe, efficient clinical pathways and governance standards.
- Collaborate with virtual wards and locality teams to streamline flow and outcomes.
Typical crisis scenarios:
- Sudden deterioration in health or wellbeing.
- Exacerbation of existing conditions.
- New or worsening presentations requiring urgent intervention in the community.
Essential criteria:
- Full GMC registration and licence to practise.
- Minimum 5 years post-qualification.
- Proven Urgent Care/UCC/Acute or Community experience (UCC experience required).
- Confident managing patients virtually and face-to-face.
- Evidenced leadership and clinical governance capability.
- Car driver with UK licence; willing to travel across the service footprint.
