We are looking for an enthusiastic Practice Nurse to join our multi-disciplinary team to provide a range of services which includes Chronic Disease Management, Cytology, Immunisations, Asthma, Diabetes Management and all tasks related to being a Practice Nurse. You would need to have experience of working in accordance with the UKCC Code of Professional Conduct and The Scope of Professional Practice. Experience of EMIS WEB clinical system is an advantage. You would need to possess excellent communication skills and can demonstrate active involvement in a general practice setting.

Main duties of the job

  • The main duties of the role are chronic disease management clinics (diabetes, asthma, COPD and hypertension), carrying out health checks and screening (cytology etc), administering immunisations and vaccinations, wound care management, travel immunisations, and any tasks related to being a Practice Nurse.
  • The Practice Nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of the Practice.
  • To be involved in health promotion, disease management, QOF and audit programmes in accordance with practice protocols and policies.

About us

Robin Hood Lane Health Centre is an exciting, forward thinking, and innovative practice in South London. We have a multi-purpose building, and have a variety of practice staff working with us. Our clinical team consists of 4 partners, 5 salaried GPs, 3 Practice nurses, an ANP, 2 Pharmacists, 2 Healthcare Assistants and Counsellors. A well established Training practice for GP Registrars, Medical Students, Pharmacist Pre-Reg and Nursing Students. We are also supported by a team of Allied Healthcare Professionals, i.e. Social Prescriber, Health Coach, Mental Health Practitioner and First Contact Practitioners. The practice is well organised with an excellent team of admin and receptionists. The Community Services are also based on site. We are a PMS Practice and have always achieved high scores in the Quality & Outcomes Framework with overall CQC rating as Good with Outstanding for services responsive to peoples needs.

Job responsibilities

  • Assist in and perform routine tasks related to patient care as directed by GPs.
  • Provide essential treatment duties such as BP checks, NHS Health checks, new patient medicals/urinalysis, prescribed injections (e.g. B12 and Denosumab), BMI and lifestyle advice.
  • Provide holistic assessment of wounds and implement wound care in line with current evidence-based guidelines.
  • Offer advice about childhood and adult immunisations and ensure vaccines/injections are administered under patient group directives or individual prescription.
  • Provide contraception advice and sexual health guidance.
  • Assist GPs with minor surgery and coil fittings, implants.
  • Request pathology tests, for example urine culture, swabs.
  • Recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines.
  • Perform Cardio-pulmonary resuscitation and the correct use of Automated Defibrillator following training.
  • Run the Anticoagulant clinic to Practice patients.
  • Work in accordance with NMC The Code for Nurses and Midwives (2015).
  • Ensure accurate notes of all consultations and treatments are recorded in the patients notes and on computers, as per NMC guidelines.
  • Maintain effective infection control measures in line with local guidelines and practice policy.
  • Ensure safe storage, rotation and disposal of vaccines and medication.
  • Assist in the completion of monthly stock take and support the cost-effective use of disposable items.
  • Chaperoning and assisting patients who are being examined by another clinician.
  • Maintain general tidiness and cleanliness of nurses and treatment rooms.
  • Participate in administrative systems in the Practice.
  • Attend and participate in weekly clinical meetings and any Practice meetings when required.

COMMUNICATION AND WORKING RELATIONSHIPS

Liaises with the General Practitioners, Practice Manager and Practice Nurses, the CCG, Hospital Doctors, Medical Secretaries, Hospital Staff and Community services.

This Job Description will be subject to periodic review and amendment in consultation with the post holder.

Person Specification

  • 2 years post registration.
  • Travel Immunisation, Childhood Immunisation, Asthma Management, Chronic Disease Management, Diabetes Management, COPD.
  • Experience of EMIS WEB system, Community Nursing, INR Experience.
  • Knowledge of UKCC professional Code/Standards, Knowledge of Clinical Governance.
  • Competent in basic nursing duties required for the post.
  • Ability to communicate verbally and in writing.
  • Ability to undertake clinical audit.
  • RGN, Family Planning Certificate, Recognised qualification to diploma level or above.
