Job summary
We are looking for a motivated and patient focused Pharmacy Technician to join our PCN Pharmacy team at Lister House Surgery.
You will play a important role in supporting safe and effective prescribing, managing medication queries, and working collaboratively across the multidisciplinary team to improve patient outcomes.

Main duties of the job
- Work as a part of the multidisciplinary team to support safe, effective, and cost-efficient prescribing, including managing medication queries.
- Support repeat prescribing, reduce medicine waste, and inappropriate poly-pharmacy.
- Manage medicines reconciliation following hospital discharges and clinical correspondence.
- Assist with medication safety, including MHRA alerts, drug monitoring, and managing medicine shortages.
- Liaise with community and hospital pharmacy teams to resolve medication-related issues promptly.
- Support medication audits, care home medicines management, and implementation of NICE and other evidence based guidelines.
- Assist Clinical Pharmacists with medication reviews, patient education, and health promotion.
- Maintain accurate clinical records, contribute to practice protocols, and ensure compliance with practice procedures.
- Provide training, mentoring, and education for pharmacy administrators, clinical staff, patients, and carers.
- Represent the pharmacy team at relevant meetings and participate in the CPPE Primary Care Pharmacy Education Pathway.

- Audit and quality improvement
- Proficient in use of Microsoft Office (including Excel, Word, PowerPoint, Teams, and Outlook)
- Experience of working to Standard Operating Procedures
Desirable

- Developing pathways or processes
- Understanding of QOF, IIF, and PCN DES
- MHRA and CAS alerts
- Understanding of Drug Monitoring and local relevant clinical and national guidance
