Would you like to join a new team in supporting individuals with trauma and mental health needs, integrating back into the community? Build trusting relationships with individuals to create a safe, supportive environment. Provide practical support with daily living, including personal care, meal preparation, and household tasks. Implement individualised support plans and positive behaviour strategies. Manage and de-escalate challenging behaviours calmly and professionally. Promote community inclusion by supporting individuals in social, leisure, and educational activities. Maintain accurate records and contribute to reviews of care plans. Work collaboratively with families, caregivers, and other professionals.
