Are you an experienced SEND professional looking for an opportunity to lead and shape outstanding transition support for learners with additional needs? We are seeking a Transition Practice Lead to oversee and develop high-quality transition processes for learners with SEND, EHCPs and other barriers to learning.
The Role
- Lead transition, assessment and admissions processes, ensuring learners receive the right support from day one.
- Coordinate and quality assure EHCP consultations and statutory processes.
- Develop effective transition and learner support plans.
- Lead and support a team of Transition Officers.
- Work closely with Local Authorities, schools, SENCOs, parents/carers and external partners.
- Support EHCP annual reviews and ensure compliance with the SEND Code of Practice.
- Monitor learner needs, provision and High Needs funding.
- Drive high-quality, inclusive practice across the organisation.
About You
- Experience working with SEND learners in an education or relevant support setting.
- Knowledge of EHCPs, SEND transition and the SEND Code of Practice.
- Experience coordinating EHCP consultations or statutory processes.
- Previous line management/team management experience.
- Experience working with Local Authorities, schools, SENCOs and families.
- Strong communication, organisation and relationship-building skills.
- A relevant qualification in SEND, education, wellbeing or a related area, or willingness to work towards one.
- Level 2 English and Maths, or willingness to achieve.
