We are looking for an experienced Paramedic Practitioner to join our new Enhanced Care & Frailty Team at Dover Town PCN. This is a multi-site role delivering high-quality, person-centred care through home visits and care home support. You will play a key role in proactive care, working closely with our multidisciplinary team to improve outcomes and independence for patients across our PCN area. Main duties of the job include:
- Undertaking both pre-arranged and same day visits to housebound community-based patients and to those living in care settings.
- Providing clinical assessment, diagnosis and treatment of patients within their scope of practice.
- Conducting regular visits to care and nursing homes, delivering proactive and responsive care in line with the EHCH framework.
- Completing initial reviews with 7 working days for new residents and hospital discharges.
- Using sound clinical judgment, prioritising caseloads, and referring for investigations as required.
- Using sensitive, person-centred communication styles to ensure patient understanding and consent.
- Promoting self-care, patient education, and preventative approaches to health and wellbeing.
- Working closely with GPs, nurses, pharmacists, social prescribers, and liaising with the wider PCN team.
- Participating in weekly Multi-Disciplinary Team meetings to support care planning for complex or vulnerable patients.
- Supporting proactive management of long-term conditions, frailty, and palliative care needs.
- Evaluating treatment plans and offering advice to care home staff.
- Maintaining professional registration and engaging in ongoing CPD and supervision.
- Participating in training, reflective practice, and relevant courses or conferences.
- Promoting equality and diversity in all interactions.
- Respecting the privacy, dignity, and beliefs of patients, families, and colleagues.
- Engaging in annual performance reviews and maintaining a professional development portfolio.
- Supporting innovation and helping develop new models of care in collaboration with colleagues and line managers.
Qualifications required:
- BSc in a training programme approved by the College of Paramedics.
- Active and relevant professional registration with HCPC.
- Completed two-year Consolidation of Learning period as a "newly qualified paramedic" AND have a further three years' experience as a band 6 (or equivalent) paramedic.
- Commitment to obtaining the clinical pillar competencies of the paramedic FCP/AP roadmap to practice within six months of the commencement of the role.
- Evidence of verification against the paramedic primary care roadmap.
- Independent or supplementary prescribing qualification, or willingness to undertake.
Experience required:
- Experience in managing the care of patients presenting with undifferentiated undiagnosed conditions.
- Strong clinical assessment and decision-making skills.
- Experience of working within a multi-disciplinary team.
- Compassionate, adaptable, and community-focused approach.
- Ability to write comprehensive, accurate clinical notes.
- Good IT skills including ability to record accurate clinical notes.
- Excellent interpersonal, influencing and negotiating skills.
- Understanding of Confidentiality and the Data Protection Act.
