Join our team as a Practice Nurse at The Dove Medical Practice and be part of a supportive, patient-focused environment. You will work collaboratively within a multidisciplinary team to deliver a wide range of clinical services, including chronic disease management, immunisations, cervical screening, wound care, and health promotion, while developing meaningful relationships with patients. Main duties of the job

Working Pattern: Monday 8.30am to 6.00pm, Tuesday OFF, Wednesday 8.30am to 6.00pm, Thursday OFF, Friday 8.30am to 6.00pm. 30 days of annual leave + Bank holidays.

As a Practice Nurse, you will deliver high-quality, patient-centred care across a range of clinical areas within general practice. Your day-to-day work will include supporting patients with long-term conditions such as asthma, diabetes and cardiovascular disease, carrying out routine reviews, and promoting self-management and healthy lifestyles. You will be responsible for providing core nursing services including immunisations, cervical screening, wound care, and health checks, as well as supporting infection prevention and control within the practice. You will work closely with GPs, healthcare assistants and the wider multidisciplinary team to ensure safe, effective and coordinated care for patients.

The role also involves maintaining accurate clinical records, supporting recall and screening programmes, and contributing to quality improvement initiatives such as QOF. You will play an important part in ensuring patients have a positive experience of the practice, offering compassionate care and clear communication at all times. Job responsibilities

  • Clinical Care & Patient Management
    • Undertake consultations and assessments of patient need, including patients and their families.
    • Deliver evidence-based nursing care in line with NICE, national service frameworks, and local protocols.
    • Support the management of long-term conditions, contributing to quality improvement and assurance initiatives.
    • Provide wound care, including ulcer management and Doppler assessments.
    • Deliver travel health advice and services.
    • Support clinical emergencies as required.
    • Recognise own limitations and escalate or refer patients appropriately.
  • Prevention, Screening & Public Health
    • Support and deliver immunisation, vaccination and cervical screening programmes.
    • Provide opportunistic health promotion and lifestyle advice.
    • Lead and support public health campaigns (e.g. flu clinics).
    • Embed population health management approaches, addressing health inequalities and improving access.
  • Care Coordination & Vulnerable Patients
    • Support the development and delivery of care programmes for frail and housebound patients.
    • Liaise with external services and agencies to ensure appropriate support for vulnerable patients.
  • Clinical Governance & Quality Improvement
    • Participate in clinical audits, research and service improvement initiatives.
    • Contribute to the development, review and implementation of practice protocols, policies and procedures.
    • Support the Infection Prevention & Control (IPC) lead and adhere to IPC policies.
    • Maintain accurate and contemporaneous clinical records using SNOMED CT coding.
    • Contribute to achieving practice targets, including QOF and enhanced services.
    • Support call and recall systems and maintain chronic disease registers.
  • Education, Training & Support
    • Teach and advise patients and their families on self-management of conditions.
    • Support and mentor new-to-practice nurses and other healthcare professionals.
    • Undertake all mandatory training and participate in ongoing professional development.
    • Participate in annual appraisal and regular performance reviews.
  • Safeguarding & Professional Responsibilities
    • Work in accordance with safeguarding policies for children and adults.
    • Be aware of and comply with current legislation, policies and procedures.
    • Participate in complaint handling and contribute to learning from incidents and near misses.
  • General Responsibilities
    • Maintain a clean, safe and effective working environment.
    • Chaperone patients where required.
    • Support the wider practice team in day-to-day operations.

Person Specification

  • Skills and Knowledge
    • Competence in Practice Nurse clinical skills (e.g. wound care, ECG, venepuncture, immunisations, new patient checks, women's health including cervical cytology and contraception).
    • Ability to request pathology tests and manage results, including patient communication.
    • Use of EmisWeb.
  • Experience
    • Broad knowledge of general practice and experience as a Practice or Community Nurse.
    • Experience of chronic disease management.
    • Experience of infection prevention and control.
    • Experience of managing elderly and housebound patients.
    • Experience of quality improvement initiatives (e.g. audit, benchmarking).
    • Understanding of the NHS landscape, including PCNs and commissioning structures.
  • Qualifications
    • Registered Nurse with Nursing and Midwifery Council registration.
    • Evidence of working at the appropriate level within the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
    • Mentor or Teaching Qualification.
    • Family Planning Qualification.
