We are looking for a compassionate, organised and experienced Care Assessor to join our growing team. This is a varied and rewarding role where you will play a vital part in ensuring people receive high-quality, person-centred live-in care in their own homes.
As a Care Assessor, you will be responsible for assessing new live-in packages, completing client reviews, and ensuring care plans accurately reflect individual needs. As we operate across England, you must be willing and able to travel and work across different regions.
About the Role
As a Live-in Care Assessor, you will often be the first face of our service for clients and their families. You will build professional relationships with clients, loved ones/representatives and professionals, ensuring each live-in package is set up safely and effectively.
This role is office (Solihull), community, and travel-based; therefore, a full UK driving licence and access to your own vehicle is essential.
Key Responsibilities
- Carrying out new client assessments for care packages
- Completing regular client reviews for existing clients
- Writing and updating person-centred care plans
- Conducting risk assessments and medication reviews
- Supporting safe set-up of packages (including liaising with family, professionals and internal teams)
- Monitoring and maintaining the quality of care delivered
- Providing guidance and support to care staff regarding client needs and best practices
- Supporting service delivery when required
- Participating in the service\’s on-call rota (where applicable)
Experience & Requirements
- Right to work in the UK
- Full UK driving licence and access to a vehicle (essential)
- Willingness to travel long distances across England
- Previous experience in Domiciliary / Live-in Care / Complex Care (desirable)
- Minimum NVQ/QCF Level 3 in Health & Social Care (or equivalent)
- Experience writing care plans (desirable)
- Experience completing assessments, reviews and risk assessments (desirable)
- Strong organisational and time management skills
- Excellent communication and problem-solving abilities
- A genuine passion for delivering high-quality, person-centred live-in care
