Douglas Macmillan Hospice in Stoke-on-Trent is seeking a Senior Health Care Support Worker to join the HART Team and support our NEW virtual ward, delivering safe, timely and compassionate care at home.
You will undertake patient observations, support care plans, provide telephone and face-to-face support, and work with a multidisciplinary team to coordinate patient access and flow across the Virtual Ward and wider community services.
