The Primary Care Provider is an employee of DotHouse Health under the direct supervision of the Medical Director or designee. The provider is responsible for direct patient care at DotHouse Health and manages the continuity of care of the patients that he or she sees or for whom he or she is responsible. Additionally, the provider is responsible for any patient related administrative work and is required to participate in departmental meetings.

Implicit in the spirit of this job description is an expectation that the provider will commit to conduct him/herself in a highly professional manner in all formal patient and staff interactions, to attend punctually all clinical sessions and meeting obligations, and to demonstrate leadership attributes as appropriate to the responsibilities assigned in the Primary Care Department.

GENERAL DUTIES & RESPONSIBILITIES:

  • Full-time is defined as 8, on-site primary care sessions. Each session is 5-hours (4 hours providing direct clinical care and 1 hour of administrative time).
  • Clinical session time will consist of direct patient care which will include at least one evening session per week and on average one Saturday AM session per 6 weeks.
  • Each patient care clinical session will include scheduled patient care time and will generate follow-up including but not limited to review of and development of appropriate response to laboratory tests results, X-rays results, and results from other studies, as well as addressing communications from patients, specialist consultants, and other members of the care team, in accordance with the Health Center's policies and protocols.
  • The performance of this follow-up will be the responsibility of the provider.
  • Direct patient care does not include inpatient hospital rounding.
  • The provider is required to participate in the Health Center's on-call system.
  • The provider is expected to be present for all scheduled clinics and will adhere to Health Center time off policies.
  • The provider will collaborate with Medical Leadership to meet the Health Center's needs for clinical coverage (e.g., coverage for vacations, sick time, and holidays).
  • Basic administrative responsibilities will include participation in meetings, trainings, and performance improvement activities as determined by Medical Leadership.
  • The provider will participate in peer review/staff evaluations and other reasonable tasks as determined by the Chief Medical Officer or designee.
  • The provider will engage in special administrative project(s) as mutually agreed upon by the APP and the Chief Medical Officer or designee.
  • Additional responsibilities may include supervision of advanced practice providers, such as nurse practitioners and/or physician's assistants, as mutually agreed upon by the Physician and the Chief Medical Officer or designee.
  • The provider will engage in the instruction of Health Center staff, students, and resident physicians as mutually agreed upon by the provider and the Health Center.
  • In some cases, the Physician may have a reduction of scheduled patients per session to accommodate teaching responsibilities.
  • The provider must be licensed to practice medicine within the Commonwealth of Massachusetts and must be eligible for credentialing at Boston Medical Center, and where appropriate, at other hospitals, and with any insurer contracting with the Health Center.
  • The provider shall participate in regular continuing medical education activities.
  • The provider will understand and fulfill all duties as outlined in the Provider Employment Agreement.

REQUIRED SKILLS & COMPETENCIES:

  • Education: Must be a graduate of an approved medical school and an accredited residency program in family medicine.
  • Certification/Licensure: Board certified or Board eligible in appropriate discipline. Must be board certified at hire. Licensed or eligible for licensure to practice medicine in the State of Massachusetts. Eligible to obtain and maintain a federal and state narcotic registration.
  • Experience: Strong community health/public health orientation; experienced in patient care management in the pediatric population; knowledge of or experience with healthcare information systems; familiarity with Epic EMR; familiarity with or experience working in community health centers. Experience working with historically underserved communities.
  • Preferred Languages: Spanish, Vietnamese, Haitian Creole, Cape Verdean Creole.
