We are looking to recruitanenthusiastic,dedicatedand reliableSupport Time Recovery Worker,who wantsto work with our dynamic and supportive team.STRW's arean important componentof the Assertive Outreach Team (AOT)service model and provide a valuable source of support to both the patients and practitioners.The role provides the opportunityand timeto use your skills todevelop rapport andtrusting relationships with patients, which is at the core of the team's approach., We are looking for a dedicated and resilient Support Worker to take a lead focus on supporting patients with co-occurring addictions (dual diagnosis).
You will provide support, give time to an allocated group of service users and thus promote their recovery and maintain them in their community environment. Within the context of this assess, plan, promote and review individualised activity plans utilising team procedures.
You will need to bring some experience of working with peoplewithSMIin a variety ofsettings,including aperson'scommunity and homeenvironment.Consideration will also be given to applicants who candemonstratetransferrable knowledge and skills, for instance gained in an in-patient setting.
The AOT employs a whole team approach. Along with other members of the team you will be involvedin discussing,formulatinganddelivering individualisedplans in the community. You will need to be able to work creatively and flexibly to deliverahigh standard ofcare thatpromote a person's ownhealth,well-beingand recovery.
As the service covers the geographical area of Bournemouth, Christchurch and Poole, the ability to drive your own vehicle is essential, please ensure you have a full and valid UK driving licence and are able to travel using your own vehicle on Trust business. You may also be expected to transport patients on occasions., You will have knowledge of the Mental Health Act and the Mental Health Capacity Act.
You will be experienced in a wider advisory capacity, such as CQUIN9 - Smokes stop and Alcohol IBA.
You will be ableto deal calmly with potentially challenging situationsand people, e.g. verbal/physicalaggression in a clinical setting.
You will have excellent communication skills in written and spoken English to a standardwhich enables the post holder tocarry out the full range of dutiesand responsibilities of the roleeffectively andbe competent working with patients, carers, families, MDT'sand a wide range of agencies.
You will be able to carry out extensive physical observations without supervision.
