We have 2 part time band 4 community nurse associate posts in the Blandford , Gillingham and Blandford locality District Nursing teams. You will deliver planned care to patients in the community setting.
Will support Individual development and training needs for the role and responsibilities. To work effectively within a large team of mixed skills when caring for complex patients in the community setting. Effective communication is key to this role and being aware of multi disciplinary working.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

To deliver planned care to a group of patients working within the standards of proficiency outlined by the Nursing and Midwifery Council (NMC) and in accordance with Trust policies and protocols
To demonstrate the ability to work effectively as part of a team and provide support/guidance to less experienced staff/learners within the clinical environment as needed
Manage a defined group of patients/service users under the indirect supervision of a Registered Nurse, using evidence based/client centred principles to undertake a range of delegated tasks independently
Contribute to the ongoing assessment, planning, implementation, and monitoring of patient/service user care, and escalate to senior registered practitioner as appropriate
To promote health and prevent ill health in service users, contributing to integrated care
To take responsibility for planned/defined tasks as require
To plan and undertake clinical tasks guided by standard operating procedures and protocols, within the limitations of the Registered Nursing Associate role.
Undertakes delegated clinical skills within their scope of practice having once undertaken relevant training, deemed competent and adhere to local and national guidance.
