An exciting opportunity has arisen for a Band 6 specialist nurse within the Stoma Care team. Recent and relevant experience in stoma care / surgical nursing is essential.
We are looking for an enthusiastic forward-thinking and motivated individual to join our Stoma Care team. If you have a passion for problem solving and improving outcomes for new stoma patients we want to hear from you!

Main duties of the job
The successful applicant will responsible for pre and post op stoma teaching for patients. Providing patients with the best opportunity for independence and confidence in managing their stomas on discharge.
This role will include working with patients on the inpatient wards as well as daily outpatient clinics.
There will also be an element of providing teaching and education for staff.

Detailed job description and main responsibilities
- Assess, plan, implement and evaluate individual patient care.
- To utilise advanced nursing skills and extensive knowledge base to undertake specialist assessments, critically analysing complex information to make informed decisions regarding appropriate treatment/intervention to ensure the delivery of highly specialist care.
- Coordinate patient centred care from diagnosis onwards.
- Communicate effectively with patients, visitors and the multidisciplinary team.
- Promote the health and empowerment of patients.
- To review patients following discharge by maintaining telephone, clinic and home visits for as long as required and ensure that ongoing support is in place as necessary.
- To support the other multidisciplinary teams in the provision of high quality stoma management for patients at Dorset County Hospitals.
- Promote and participate in multidisciplinary team working – attend MDT meetings and communicate and coordinate information obtained.
Person specification
EDUCATION, QUALIFICATIONS & TRAINING
- Registration on Part 1 or 12 of NMC Register (RGN or RN Adult).
- Post Registration Qualification or experience in this specialty
- Teaching/Assessing Qualification
- Evidence of recent, relevant professional and personal development.
KNOWLEDGE & EXPERIENCE
- Significant relevant post registration experience in an acute setting
- Significant recent post registration experience working within a Stoma care service.
- Experience of motivating and supporting a team.
- Experience of providing training.
- Experience of change management
- Awareness of applied anatomy and physiology in related area
- Experience of presenting to others.
- Experience of managing resources.
- Awareness of standard setting and clinical governance (research, audit and quality improvement).
- Experience of supporting patients with chronic conditions.
SKILLS & ABILITIES
- Demonstrate a clear understanding of stoma management, operations, care and trouble shooting.
- Demonstrate an understanding of the issues for a patient living with a stoma.
- Demonstrate an understanding of Information Governance in relation to the Stoma Care Team
- Evidence of ability to maintain accurate and legible documentation, both written and via computer systems which includes local databases and the Coloplast Mime Patient Record System.
- Evidence of ability to maintain effective working relationships with colleagues, patients and visitors
- Ability to motivate self and others.
- Decision making skills-both clinical and non-clinical
