About The Role
As a Keyworker, you will provide personalised, community-based support to children and young people aged 0-25 with autism and/or learning disabilities who are at risk of mental health hospital admission or are currently in inpatient settings. Your role will be pivotal in:
- Building strong, trusting relationships with children, young people, and their families/carers.
- Acting as a key point of contact to coordinate care and support.
- Supporting the implementation of personalised plans.
- Working across services to remove barriers and improve outcomes.
- Participating in Care, Education, and Treatment Reviews (CETRs).
What You’ll Do
- Provide direct support to children and young people on the Dynamic Support Register (DSR).
- Collaborate with multi-disciplinary teams across health, education, and social care.
- Contribute to service development and evaluation through data collection and feedback.
- Attend and contribute to national and regional Keyworker Community of Practice events.
Travel Requirement
This role involves significant travel across the Dorset Council area. A suitable vehicle must be available on most working days.
